 This position would be a supervisory position in Supported Living requiring every third weekend on-call.

GENERAL RESPONSIBILITIES:

Performs all the functions of a DSP’s and assists the Program Coordinator in supervising, supporting and training DSP’s and in ensuring quality of services provided and the health and safety of the individual served.

ESSENTTIAL FUNCTIONS AND RESPONSIBILITIES:

  1. Collect and disseminate forms and agency information as delegated by supervisor.

  2. Assist with training and monitoring DSP’s.

  3. Assist with:

  • Review of goals and preparation of goal sheets

  • Planning and follow-through of doctor appointments

  • Updating client files

  • Planning living arrangements for clients

  • Moving clients into their new homes

  • Assisting in crisis situations

  • Meeting with diagnostic and evaluation team

  • Review and checking in medications

  • Assist with medical emergencies

  • Assist with quarterly reports

  • Assist with coordinating staffing coverage

  • Track and monitor employee CPR/FA, Auto Insurance, D.L., OSHA/SAT, Mantoux

  • Complete home observations

  1. Assist in finances with:

  • Monitor in home client spending money and staff petty cash

  • Write checks for living expenses if applicable
  • Cash checks and make deposits if applicable

  • Complete paper work for grocery bills and write checks, if applicable

  1. Help develop and manage staffing schedule in accordance with Plan of Care.

  2. Responsible for ensuring that the individual’s home is safe, clean and in good repair.

  3. Help collect and manage individual’s financial information.

  4. Assist with in home client finances and money management.

  5. Assist in monitoring individual’s health care needs

  6. Monitor proper diet, meal planning and shopping.

  7. Ensure that monthly safety drills are conducted and documented.

  8. Must report any suspected abuse, neglect or exploitation immediately to supervisor or department head.

  9. Attend monthly department meetings and conduct at a minimum monthly home meetings

12. Maintain documentation, certification and training as required to comply with federal, state and program requirements and standards.

  1. Perform other duties as assigned.

  2. Comply with all standards to assure the health and safety of all staff and individuals we serve.

  3. Report to work as scheduled.

KNOWLEDGE, SKILLS, AND ABILITIES:

  1. Effectively train new employees

  2. Organization and ability to multi-task

  3. Ability to manage client finances

  4. Effectively communicate with all internal and external stakeholders

QUALIFICATIONS:

  1. Valid driver’s license

  2. Experience working with people with developmental disabilities preferred

  1. High school diploma or equivalent

  2. Must be at least eighteen (18) years of age
