Job Description

POSITION TITLE: Program Coordinator

REPORTS TO: Director

SUPERVISES: Supported Living Supervisors, Personal Assistants

GENERAL RESPONSIBILITIES:

Ensures quality, health and safety and client centered approaches for individuals in community living services. Supervises, supports and trains staff as they assist individuals served. Ensures compliance with federal, state and local regulations while expanding opportunities for persons served to participate in community life as they choose.

SPECIFIC RESPONSIBILITIES:

  1. Develop and implement the Individual Support Plan and assist in development of the Individual Supported Living (ISL) budget for each person served.

  2. Influencing and procuring an ISL budget that addresses all client needs and articulating elements to appropriate staff.

  3. Assessment, planning, implementation, and review of person-centered support plans for people served.

  4. Coordinate the delivery of all services provided by Benchmark Human Services and the community.

  5. Assist people served in expanding their opportunities to participate in community life as they choose.

  6. Providing on-call direct support and coverage coordination.

  7. Participate in the hiring, training and supervision of Supported Living Supervisors and Personal Assistants.

  8. Schedule staff in accordance with ISL budget and services identified on the Individual Support Plan and providing emergency coverage.

  9. Accurate and timely completion of ISPs, Monthlies, Addendums, client financials, QA walk throughs, monthly and daily documentation (logs, fire and tornado drills, client medical information, MARs, etc.)

  10. Be aware of and submit reportable events within required time frames.

  11. Manage and monitor client finances within required time frames

  12. Serve as liaison with service coordinators, families, other agencies and internal programs.

  13. Work collaboratively with director, nurse, HR, accounting, IT, etc.

  14. Perform other duties as assigned.

  15. Comply with all standards to assure the health and safety of all staff and clients we serve.

QUALIFICATIONS:

  1. Bachelor’s degree in human services and one (1) year experience working with developmentally disabled people

  2. Valid drivers license

  3. Access to vehicle in good operating order
